They seem to be performing surgery on the bag, not her. And this is one where the "illustration meets photography" isnt helping much.

I like this one because the two situations, settings could have been similarly composed. The surfer is less clever - just a guy standing on a beach with or without a plaster.
That the lady is too sexy is distracting. And the doctors look too ghoulish and Faustian.
