Personally... I would've made the door handle and lock (etc) look more like genitalia - and leave the 'glory hole/peep hole' at normal standing height.
Less about cliched 'gay sex' ideas and more about clever.

I think this works a few ways. It could mean the people are on the floor having sex so that's what you'll see, or the obvious bend over to look gag. Not super clear to me. But it's alright.
