Isn't the ear made the other way round? IF you look at it, the neck is pointing right, whilst the ear left. Even though, it's a cool add, which with the white tonality emphasizes the product, It's like the black&white concept for conditional advertising.

Anatomically speaking, the ear has juxtaposed where the navel should be, and the organ (male or female) between the legs has been nicely retouched and removed..
