problem is there & the solution is right next to it
the moment you see something like this, you connect to it. you say ok , it feels exactly like this when you get mouth ulcer.
and the people who understand the problem have come up with the solution. that compels the reader to try the product once
It is nice to see your logic, but you need to see things in parallel structure when you are critiquing. What I mean is, If I try another execution of this by using a photo of a box of push pins, is it good? No. Taking and putting those push pins on a plate is an avenue for "Hey, you eat things on plates" – but that doesn't mean much at the end of the day. It doesn't force your viewer to "ingest" your ad – no pun intended.
On the brighter side, you are thinking of problem solution and simple relationships. Pursue this further and I think you'll have something. Just be careful, great work has been done in this category.

I think the idea is great. I would rather use needles or pins. And, of course do better photography.

For mouth ulcer…bonjela gels; http://www.drugdelivery.ca/s33758-s-BONJELA-GEL.aspx are very helpful. When I suffer from mouth ulcer I use this gel; it provides immediate relief. If you planning to try it then let me tell you that the moment that you’ll apply it on the affected are you’ll experience extreme pain but it will last only for some seconds….after that you’ll get complete relief.

The ad is missing focus right now, the plate is half off and the fork is too high up. Just shoot a plate with nails and perhaps all sorts of sharp things and let that be at the centre or just shoot the fork and give it some space. Maybe show it in a dining setting so that the content of the plate is even more out of place.
This is a great ad...if the year was 1994.

In my opinion with good art direction and better lay out this could very well work. I'd like to strongly press the op to put some more time in it and make it work and *not* starting over. Make it look like something from the front page, work hard.
I just don't think the idea is original enough.
Reminds me of the Preparation-H stuff back in 1994ish with the handsaw as the bicycle seat.
I think it totally depends on the execution, many of the 90's ads are still valid today but lack the 2000's lush lay outs and art.
