Concept is good but could be worked on.
We just don't realise the number was referring to age as well until we read the description.
Your headline says "play safe".
People would immediately relate it with a sex.
I don't think there's any need to further use the 69 as an sexual number.
It'd be nicer if the effort is spent more on the age-ending idea you had in your description.
Execution wise, maybe you should relate it with a person, a place or event where would relate more of a place where someone's life ends.

if life if short, then why play safe? viewer might need a better reason to play safe. plus, everybody knows that STDs r the reason to play safe.
If its a 'reminder' ad, just that play safe and the visual of the plastic is good enough.
If its persuasive, i say, STDs are overused. you can portray the brilliant small events that a man can miss if he suffers/dies of STDs (example, his daughter's admission to medical college, his son's marriage or his sixty-first birthday present from his wife...). Then say, 'play safe'.
