When you say something about chain, you don't have to always show a chain.
Maybe, it means the affected should stop all unprotected physical contact with anybody. This person has to volunteer to break the chain and stop spreading further. There fore the chain is a representation of continuity that has to break to avoid exposure to the innocent.

When u talk about my work, same one say's that no needs to show the chain, how it can be passable to say about continuity.

To be honest I did not understand it until You post some sort of explanation. The idea of taking some responsibility for somebody illness is quite interesting, I have to agree with alvinpck that there’s no need to show the chain. Chain is not important here - people are. Maybe you could try different approach and focus on essential things here - responsibility. Community on the other hand can be shown in different ways too. What makes community work? I think trust, faith, reliability. Maybe You could try to work with these values. Think about it.
